If you have a metal fabricating shop in your area,they can construct,fairly inexpensively I'm sure,a metal frame the size you need,out of angle iron and a cover from 3/8 or 1/2" steel. You can have them trim one side of the angle down so it's the same height as the thickness of the steel plate you use,so it will be flush. You can also have them drill a hole or 2 in the cover for easy removal if needed.

After you've built an outside and inside form,you can lay the frame into the cement. Visualize a manhole cover. Same thing,basically.
