Ceramic Tile Grout Cracking

The floor was installed in December. Grout cracked and was repaired in February. Grout has cracked again in same areas. Note that underlayment and the proper grout was used. Which of the following would cause this:

1. Flex in the floor (how much is enough?)

2. Gluing the underlayment without nailing or
screwing it down.

3. Other?

With regard to #2, short of pulling the tiles is there any way you can tell if it was not screwed down? Should the screws, nails be visible from the crawl space right below it?
